JOHN BIRCHARD

SILVER SPRING, Md. — Veteran broadcaster-writer John Birchard died Aug. 22, 2017, at home in Silver Spring, Md., at the age of 81.

A Vermont native, Mr. Birchard enlisted in the U.S. Air Force following graduation from high school in Shoreham, Vt.

VERGENNES — Carol (Bristol) Jennings of Vergennes passed peacefully surrounded by her children on Aug. 31, 2017, after a long illness. She was born to Kenneth and Kathleen (Kay) Bristol of Vergennes on July 9, 1939. Her sister Brenda predeceased her in June 2010.

Carol Jennings, 78, Vergennes

VERGENNES — Carol (Bristol) Jennings of Vergennes passed peacefully surrounded by her children on Aug. 31, 2017, after a long illness. She was born to Kenneth and Kathleen (Kay) Bristol of Vergennes on July 9, 1939. Her sister Brenda predeceased her in June 2010.

MIDDLEBURY — A memorial and celebration of the life of Katherine G. Schmidt, who died Jan. 11 at her East Middlebury home at the age of 94, will be held Saturday, Sept. 9, from 1-3 p.m. at the Champlain Valley Unitarian-Universalist Society, 2 Duane Court, Middlebury.

Emile A. Paquette

BRISTOL — Emile A. Paquette, born October 29, 1932, in New Haven, passed away at 84 on Monday Aug. 28, at his home in Bristol with his loving wife by his side.

Emile married Marjorie Cousino on July 18, 1953. They resided in Bristol and raised seven children: Anne Layn and husband David of Monkton; Emile and Elaine Paquette of New Haven; Geralyn Barrows and husband Gary of New Haven; Pat and Sage Paquette of New Haven; Tim and Melanie Paquette of New Haven; Alice Barnum and husband Charles of New Haven, and Amy Driscoll and husband Mark of Monkton.

Michele V. Forman

MIDDLEBURY — After several years of dealing with complications from Type 1 diabetes and dementia, Michele V. Forman died on August 28, 2017. Raymond F. Cousino

LINCOLN — Raymond F. Cousino of Lincoln passed away unexpectedly on Saturday, Aug. 26, 2017. He was born in Middlebury on July 13, 1957, the son of Edmund Cousino and Nancy (Sargent) Ash.

He is survived by Jayne, his wife of 37 years; sons Josh and wife Julie and Joe and wife Simona; daughter Sarah and partner Randy; Son-in-law Todd and partner Jolene; four grandchildren, Tristin, Kelsey, Caden and Everett; brothers Edward, Donnie, Paul, Johnny and Nicky, and sisters Rhonda and Paula.

Jessie Olene Booska

MIDDLEBURY — Jessie Olene Booska passed away on Aug. 21, 2017. She was born in Devereux, Ga., the daughter of Walton and Jessie (Walker) Davis.

She is survived by her daughter, Glenna Piper and her husband AJ of Weybridge, her son Gary of Middlebury and many grand and great grandchildren. She was predeceased by her husband Paul in 1982.

A graveside service will be held Saturday, Sept. 2, at 2 p.m. at the Weybridge cemetery. In lieu of flowers donations may be made to Middlebury Regional Emergency Medical Services or Addison County Home Health and Hospice.

PAUL V. ROTAX

LINCOLN — Paul V. Rotax, 85, passed away Sunday, Aug. 20, 2017, at Porter Medical Center in Middlebury.

He was born April 11, 1932, in Burlington, the son of Frederick and Bessie (Stillson) Rotax.

He has been married to Doris Rathbun Rotax for 62 years.

Paul is survived by his wife Doris; four children, Joanie O’Leary, Cindy Gebo, David Rotax and Dennis Rotax; nine grandchildren; and seven great grandchildren.

He was predeceased by son Lester Rotax and eight brothers and sisters.

JACK H. MCGARVEY

RIO RICO, Ariz. — Jack H. McGarvey passed away on July 15, 2017, at the age of 80 years. He died in his beloved home in Rio Rico, AZ. The cause was heart disease.

Submitting an obituary

The Addison Independent publishes paid obituaries and free notices of passing. Paid obituaries cost 25-cents per word, and $10 per photo. The obituary will publish as it is submitted on the date of the family’s choosing.

For those who do not wish to purchase an obituary, the Addison Independent offers anyone with a local connection the opportunity to publish a notice of passing. These notices may be up to 100 words and are subject to editing by our news department.

Submissions must be received by 11 a.m. on Wednesday for publication in the Thursday paper and 11 a.m. on Friday for Monday’s paper.

Generally, obituaries follow this form:
TOWN — Name, age, of town, died on date, of cause of death. He/She was born on date in town, the son/daughter of father's name and mother's name, including maiden name in parentheses.

Then include a paragraph or two about where the person went to school and what they did for work; where they lived, who they married and when.

Add a sentence or two about what made the person special to family and friends, including hobbies, avocations or other telling details about their life.

He/She is survived by spouse's name; children named individually (and their spouses and towns of residence, if appropriate); siblings named individually; number of grandchildren, parents or other immediate family members.

He/She was predeceased by names of immediate family members.

A memorial service will be held (fill in details of date, time, and place, plus officiate if applicable) with interment at (place). Memorial contributions may be made to (name and complete address, if applicable).
