HAROLD MAYFORTH

BRISTOL — Harold “Hal” Mayforth Jr., aged 91 years, of Bristol died Feb. 13, 2013.

JOAN ADAMS

SOUTH BURLINGTON — Joan Ann Adams, 73, died at the Vermont Respite House on Sunday, Feb. 10, 2013, with her family by her side after a short illness.

Joan was born on May 1, 1939, in Panton, the daughter of Neil and Mabel (Danyow) Bull, although Joan was raised by Reka Austin for most of her life. She attended school in Middlebury. On Sept. 6, 1958, she married George W. Adams in Middlebury and together they had four beautiful children. Although they had not been together for many years, George and Joan remained good friends.

JOANN CHILD

SHELBURNE — Joann Cutting Child, 80, of Shelburne, passed into a new life Feb. 8, 2013.

Born Oct. 16, 1932, the daughter of Charles Cady Cutting and Mildred Scarborough Cutting, Joann grew up in Middlebury. She was a seventh-generation Vermonter and a direct descendant of John Everts, who surveyed the town of Middlebury in 1761.

ALLEN QUESNEL

CORNWALL, Vt./BRACEY, Va. — Allen Leo Quesnel, age 66 years, died in the arms of his loved ones on Thursday morning, Feb. 7, 2013, at the St. Albans Road home of his daughter Kim in Swanton, Vt. Allen waged a courageous battle with male breast cancer since 2010.

Born in Middlebury, Vt., on Nov. 27, 1946, he was the son of the late Paul Leo and Ida Mae (Ryan) Quesnel. He attended schools in Middlebury and on Aug. 9, 1969, was married to the love of his life, Martha Ann (Johnson), who survives him.

PAUL E. BARROWS

BRISTOL — Paul E. Barrows, 49, passed away Wednesday, Feb. 6, 2013, at his home in Bristol.

He was born July 2, 1963, in Springfield, Mass., the son of the late George Barrows and Joan (Tatro) Barrows and companion William “Bill” Ladeau.

Paul was employed by D&M Petroleum of Monkton as a Petroleum Service Technician, a job he loved. He enjoyed going to work each day with his great coworkers.

MARY HALL

RUTLAND—Mary Elizabeth Hall, 83, died Thursday, Feb. 7, 2013, at Rutland Regional Medical Center.

She was born in Manhattan, New York City, on May 14, 1929. She was the daughter of Charles and Anne (McCaffrey) Duffy. She grew up in the Bronx where she received her early education and attended Cathedral High School. In her earlier years she worked for Ma Bell in the Bronx.

LISA SPRAGUE

MINEVILLE, N.Y. — Lisa Rae Sprague, 48, of Mineville, N.Y., died Jan. 13, 3013, at Elizabethtown Community Hospital, with her family by her side.

She was born on Dec. 14, 1964, at Elizabethtown Hospital. She was the daughter of Donald and Patricia Sprague.

A former resident of Orwell, Vt., she worked at Simmonds Precision in Vergennes, Vt., and elsewhere in Addison County. She fought multiple sclerosis for over 28 years. Her niece Stacy Anderson and her best friend, Becky Sheppard, were with her constantly during the last seven days of her life.

CASTLETON — Kenneth James Carleton, 58, died Wednesday, Jan. 30, 2013, at his home in Castleton.

THOMAS CHAMBERLAIN

STARKSBORO — Thomas Chamberlain, 68, of Starksboro, died on Jan. 30, 2013, in Lake Placid, Fla., after a long and courageous battle with pulmonary fibrosis. He maintained his love of and zest for life throughout his struggle.

He was born on Aug. 24, 1944, in Vergennes, the son of Carl and Lila (O’Bryan) Chamberlain. He was married on May 19, 1962, in Bristol to Patricia Grant.

WILLIAM SMITH

MONKTON — William Henry Smith, 85, of Monkton died Saturday, Feb. 2, 2013, at Kim and Gary Smith’s home in Bristol.

He was born Sept. 21, 1927, in Monkton, the son of Eli and Betsy White Smith.

His family says his hobbies were hunting, fishing, woodworking, and gardening. He loved animals and he had a good sense of humor.

He served in the U.S. Army during World War II in Germany.

Submitting an obituary

The Addison Independent publishes paid obituaries and free notices of passing. Paid obituaries cost 25-cents per word, and $10 per photo. The obituary will publish as it is submitted on the date of the family’s choosing.

For those who do not wish to purchase an obituary, the Addison Independent offers anyone with a local connection the opportunity to publish a notice of passing. These notices may be up to 100 words and are subject to editing by our news department.

Submissions must be received by 11 a.m. on Wednesday for publication in the Thursday paper and 11 a.m. on Friday for Monday’s paper.

Generally, obituaries follow this form:
TOWN — Name, age, of town, died on date, of cause of death. He/She was born on date in town, the son/daughter of father's name and mother's name, including maiden name in parentheses.

Then include a paragraph or two about where the person went to school and what they did for work; where they lived, who they married and when.

Add a sentence or two about what made the person special to family and friends, including hobbies, avocations or other telling details about their life.

He/She is survived by spouse's name; children named individually (and their spouses and towns of residence, if appropriate); siblings named individually; number of grandchildren, parents or other immediate family members.

He/She was predeceased by names of immediate family members.

A memorial service will be held (fill in details of date, time, and place, plus officiate if applicable) with interment at (place). Memorial contributions may be made to (name and complete address, if applicable).
