TINA FORREST

BRANDON —Tina Marie Forrest, 57, died Monday, Sept. 16, 2013, at her home in Brandon.

She was born in Rutland on March 4, 1956. She was the daughter of Raymond and Elizabeth (Ryder) Forrest. She received her early education in local Forest Dale and Brandon schools and attended Otter Valley Union High School.

RUTH LENNOX

MIDDLEBURY — A celebration of life service for Ruth E. Lennox, 94, who died July 29, 2013, will be held at St. Stephen’s Episcopal Church in Middlebury on Saturday, Sept. 21, at 11 a.m.

EDWARD F. “DICK” MENARD

LINCOLN — Edward F. “Dick” Menard, 78, one of the most unique and kind men ever known, was lifted away quietly on Sept. 13, 2013, while he slept surrounded by his family at his home in Lincoln, Vt. He was loved by many, his family will miss him, and may God greet him with open arms.

BETTE MOFFETT

BRANDON —Bette L. Moffett passed on Sept. 11, 2013, at her home in Brandon. She loved Brandon and was eternally grateful for the love and support she received from her community.

Born Bette Lou Little on May 27, 1924, in Kingsley, Iowa, she was the daughter of Jack and Lula (Miller) Little. Bette grew up on a farm and attended the University of Iowa. Out of college, she was a buyer for Marshall Field’s, in Chicago. In 1951, she married Hugh O. Moffett, an editor for Life magazine, and started a family in Port Washington, N.Y.

ELEANOR PRATT

CORNWALL — Eleanor R. Pratt, 86, of Cornwall died Sept. 12, 2013, at a private care facility in Salisbury under the care of hospice.

She was born on Jan. 6, 1927, in Shoreham, the daughter of Joseph and Ruth (Egleston) Ringey. She was a 1944 graduate of Middlebury High School. At the family home in Cornwall she married Carroll H. Pratt in 1947. She was a homemaker and raised her family. They say she loved reading, flowers, listening to music and attending Project Independence.

JOHN RUNYON

SALISBURY — A memorial service for John L. Runyon, 82, of Middlebury will be held on Saturday, Sept. 21, 2013, at 9 a.m. at the Salisbury Congregational Church, with Rev. John Grivetti officiating. He died on Aug. 17, 2013, at Helen Porter Healthcare and Rehabilitation Center in Middlebury.

SALISBURY — A memorial service for John L. Runyon, 82, of Middlebury will be held on Saturday, Sept. 21, 2013, at 9 a.m. at the Salisbury Congregational Church, with Rev. John Grivetti officiating. He died on Aug. 17, 2013, at Helen Porter Healthcare and Rehabilitation Center in Middlebury.

PATRICIA "PAT" SHAW

WESTON — Patricia Mullen Kelley Shaw passed away on Sept. 17, 2013, at her home in Weston, Vt.

She was born March 10, 1931, the youngest in a large and loving Irish family in Waterbury, Conn. She married Jack Kelley and settled in Cheshire, Conn., where they raised Lauren and lived a typical suburban life in the 1950s and ’60s. Holidays were a highlight, celebrating with her mother, Nora; brother, Father Bill; and with the Mullen, Shutts, Collins and Kelliher families.

CHARLES MALZAC

EAST MIDDLEBURY — Charles E. Malzac, 79, died Saturday, Sept. 7, 2013, at The Pines of Rutland.

Born Jan. 20, 1934, in New Haven, he was the son of the late Joseph N. and Nora (James) Malzac.

He honorably served his country in the United States Army, receiving two Purple Hearts during his service in the Korean War. 

During his youth he was an Eagle Scout in the Boy Scouts of America.

Following his disability he was self-employed at various jobs in the local area.

GERARD "JERRY" TRUDEAU

MIDDLEBURY — Gerard (Jerry) Francis Trudeau, 83, a life-long resident of Middlebury, passed away in comfort with his family present on Sept. 8, 2013.

Gerard was born Aug. 11, 1930, in Hartland, VT, son of Paul L. Trudeau and Cornelia H. Trudeau (Herrington).

Submitting an obituary

The Addison Independent publishes paid obituaries and free notices of passing. Paid obituaries cost 25-cents per word, and $10 per photo. The obituary will publish as it is submitted on the date of the family’s choosing.

For those who do not wish to purchase an obituary, the Addison Independent offers anyone with a local connection the opportunity to publish a notice of passing. These notices may be up to 100 words and are subject to editing by our news department.

Submissions must be received by 11 a.m. on Wednesday for publication in the Thursday paper and 11 a.m. on Friday for Monday’s paper.

Generally, obituaries follow this form:
TOWN — Name, age, of town, died on date, of cause of death. He/She was born on date in town, the son/daughter of father's name and mother's name, including maiden name in parentheses.

Then include a paragraph or two about where the person went to school and what they did for work; where they lived, who they married and when.

Add a sentence or two about what made the person special to family and friends, including hobbies, avocations or other telling details about their life.

He/She is survived by spouse's name; children named individually (and their spouses and towns of residence, if appropriate); siblings named individually; number of grandchildren, parents or other immediate family members.

He/She was predeceased by names of immediate family members.

A memorial service will be held (fill in details of date, time, and place, plus officiate if applicable) with interment at (place). Memorial contributions may be made to (name and complete address, if applicable).
