ADDISON COUNTY — A bad crash in Addison early Sunday morning sent three people to the hospital — one with serious injuries.

On Sunday, March 27, at approximately 2:41 a.m. Vermont 911 received a report of a motor vehicle crash with injuries at the Jersey Street South and Route 17 intersection in Addison.

 Vermont State Police, Addison Volunteer Fire Department, Middlebury Regional EMS and Bristol Rescue Squad personnel responded to the scene. Upon arrival, responders observed a 2006 Chevrolet truck off the roadway, in a tree line, adjacent to a field. The truck had sustained significant front-end damage.

 The state police trooper said his preliminary investigation indicated that 21-year-old DJ Herriman of Brandon was driving the truck northbound on Jersey Street South when it crossed over Route 17, left the roadway and smashed into a line of trees. The weather was clear and the road was dry.

Herriman, who was reportedly not wearing a seatbelt, sustained serious internal injuries. First responders extricated the driver from the vehicle and transported him to the University of Vermont Medical Center. Herriman’s two passengers were transported to Porter Hospital in Middlebury, where they were treated and released.

On Tuesday afternoon, UVM Medical Center reported that Herriman was in “fair” condition, which is between “good” and “serious,” and is described as, “vital signs are stable and within normal limits; patient is conscious, but may be uncomfortable; indicators are favorable.”

State police said the crash was still under investigation, but they determined that speed, alcohol and seatbelt usage were contributing factors in this crash.

In other recent activity, state police troopers:

• On March 22 at 2:33 p.m. located 24-year-old Lindsay Cushing of Middlebury, who was wanted by police on an attempting to elude law enforcement charge. The trooper, who found Cushing on Mount Independence Road in Orwell, took Cushing into custody and transported her to the Addison County Criminal Court.

• Stopped a vehicle on Case Street in Middlebury at 11:43 p.m. on March 23 and ended up arresting the driver, identified as 47-year-old Anthony E. Andrews of Middlebury on an active arrest warrant. Andrews was lodged at the Chittenden Regional Correctional Facility; bail was set at $262.

• On March 23 at approximately 11 p.m. stopped a motor vehicle on Plank Road in Bristol and cited Timothy L. Taylor, 50, of Bristol for driving with a criminally suspended license.

• On March 25 at 6:38 p.m. stopped a vehicle on Route 7 in Leicester after observing a traffic violation. A preliminary breath test showed that the driver, identified as 56-year-old Deborah Duval of Brandon, had a blood alcohol content of 0.098 percent; the legal limit for driving is 0.08. The trooper cited Duval for driving under the influence.

• On March 26 at 15 minutes past midnight stopped a vehicle on Creek Road in Middlebury, and, after a short investigation, determined that the driver was under the influence of alcohol. A preliminary breath test showed that Dakota Yankton, 31, of Bristol had a blood alcohol content of 0.178 percent — more than twice the legal limit. Police cited Yankton for DUI.

• On March 26 at just before 9 p.m. stopped a vehicle on Lower Notch Road in Bristol for a traffic infraction. Police identified the driver as Michael Desjadon, 23, of Salisbury, and then determined that Desjadon was in violation of his conditions of release as he was not supposed to be driving. Police cited Desjadon for violation of conditions of release.

• On March 28 at a little before noon, state police were called to a verbal dispute at the McDonald’s restaurant on Route 7 in Ferrisburgh. Police report that Daryn T. Barsalou, 20, of Bristol was involved in the dispute, but he left the McDonald’s before troopers arrived. Barsalou voluntarily came to the state police barracks in New Haven, where he was taken into custody and cited for violating an abuse prevention order and for violating conditions of release by being at the McDonald’s and having contact with a juvenile on the premises.

State police lodged Barsalou at the Chittenden Regional Correctional Facility; bail was set at $500.

This followed an incident at 9 p.m. the previous evening at the McDonald’s in Middlebury, where Middlebury police cited Barsalou for reckless endangerment, disorderly conduct and violating the terms of his release. Middlebury police alleged that Barsalou pointed a gun at another person at the restaurant.

• On March 28 announced that the Department of Public Safety and the Vermont Department of Labor agreed to resolve the investigation into the Vermont State Police physical training procedures for its Tactical Services Unit that resulted in the death of Trooper Kyle Young.

Young, a 28-year-old Monkton resident, died suddenly of heatstroke during a Sept. 17 Tactical Services Unit tryout exercise at a firing range in Jericho.

In the announcement, Keith Flynn, commissioner of the Vermont Department of Public Safety, said that the Vermont Occupational Safety and Health Administration acknowledged his department’s commitment to improving its training and physical testing protocols to better protect its members.
